Apex Hospitals Surgeons Successfully Treat Rare Pediatric Jaw Fracture with Modified Gunning Splints
Mumbai: Apex Hospitals has successfully managed a rare and complex pediatric mandibular fracture in a 4-year-old boy, marking a significant milestone in minimally invasive maxillofacial surgery. The child, Master chinmay patil (name changed) suffered a displaced condylar process fracture with temporomandibular joint (TMJ) dislocation following a fall while playing.
The surgical team, led by Dr. Vinod Pal, MDS (Oral & Maxillofacial Surgery), Fellowship in Head & Neck Oncology and Reconstructive Surgery, performed a closed reduction with intermaxillary fixation (IMF) using modified Gunning splints—a conservative technique that avoids open surgery and helps preserve the child’s natural jaw growth. He was assisted by Dr. Gaurav Naik, MDS (Oral & Maxillofacial Surgery), Implantologist, with anesthesia care provided by Dr. Sumeet Warang, MBBS, DA, Fellowship in Pediatric Pulmonology.
Imaging had revealed an antero-medially displaced condylar process fracture, raising concerns of long-term facial asymmetry and impaired jaw function. The modified Gunning splints, customized for pediatric use, were applied to stabilize the jaw for two weeks, followed by elastic guidance for one week.

Post-surgery, the child was placed on a soft/liquid diet and began gentle physiotherapy after splint removal. Within weeks, he regained normal mouth opening and chewing function. Follow-up at three months confirmed complete functional recovery with no deviation of the mandible.
