Thiruvananthapuram: Doctors at KIMSHEALTH successfully performed a high-risk liver transplant on a 19-year-old , from Thiruvananthapuram, who was suffering from Severe Hepatopulmonary Syndrome (SHPS), a serious complication of chronic liver disease, that caused his oxygen saturation level to plummet to just 24 percent. Severe Hepatopulmonary Syndrome is a condition in which the blood vessels in the lungs become wide, impairing the flow of oxygen to the body. To date, a liver transplant on a patient with SHPS and such profound oxygen desaturation, has not been documented in liver transplant literature.
The patient’s ailment left him dependent on round-the-clock oxygen support, forcing him to carry an oxygen cylinder on his shoulder every time he needed to get out of bed. As his health continued to deteriorate, a liver transplant became the only option for survival. Recognising the complexity and risks involved, the medical team led by Dr. Shabeerali T.U, Senior Consultant and Director-Transplant Services, and Dr. Shiraz Ahmad Rather, Clinical Chair and Lead G I, HPB and Multi-organ Transplant Services, performed a formidable twelve hour long transplant.
“Liver transplant on a patient with severe hepatopulmonary syndrome poses significant complexities, particularly due to the associated respiratory complications. Successful management requires meticulous planning, precise surgical execution, and exceptional pre and post-transplant critical care. This multidisciplinary effort was led by Dr. Hashir A, and Dr. Abhijith Uthaman, Consultants, Dr. Prijith R.S and Dr. Rachana C. Nair, Associate Consultants, Anaesthesiology, Dr. Ajay Ravi, Consultant, Pulmonology, and the nursing team,” said Dr. Rather.
Post transplant, the patient was in the hospital for three months, and was discharged following a significant improvement in his respiratory status, with no further requirement for oxygen support. His liver functioning is also back to normal. Dr. Varghese Yeldho, Consultant, Hepatobiliary, Pancreas and Liver Transplant, Surgery, Dr. Madhu Sashidharan, Senior Consultant, Gastroenterology, Dr. Manish Kumar Yadav, Senior Consultant, Interventional Radiology, and Dr. Manoj K.S, Senior Consultant, Imaging and Interventional Radiology, were also part of the treatment.
